ABSTRACT

O objetivo com este estudo foi avaliar o conhecimento de enfermeiros intensivistas sobre a importância da infusão contínua de catecolaminas. Trata-se de estudo descritivo, exploratório com delineamento transversal, desenvolvido em Unidades de Terapia Intensiva de um Hospital Escola no município de Fortaleza-CE. Foram entrevistados 49enfermeiros. A maioria, 80,4% (40), declarou, ainda, que não tinha o título de especialista em terapia intensiva. Os dispositivos mais utilizados para a infusão contínua de catecolaminas foram bombas infusoras e equipos com injetores laterais (93, 88%). Dado preocupante foi encontrado quando 54,34% dos entrevistados relataram que não obedeciam rigorosamente à infusão contínua. Em relação à investigação do conhecimento dos enfermeiros sobre a importância do conceito farmacológico desse tipo de infusão, 57,14% da amostra demonstrou dominar de forma parcial a importância farmacológica do conceito de infusão contínua. Conclui-se que a elaboração de protocolos especificando a necessidade de troca continuada das soluções possa auxiliar significativamente a assistência de enfermagem.


The aim of this study was to evaluate the nurses’ knowledge about the importance of continuous infusion of catecholamine. This is a descriptive exploratory cross-sectional study conducted in the Intensive Care Unit of a teaching hospital in Fortaleza-Ceará. We interviewed 49 nurses. The majority 80.4% (40) were not specialists in intensive care.The most used device for the continuous infusion of catecholamines were infusion pumps and jet-directed catheter(93. 88%). Worryingly 54.34% of the nurses interviewed stated they do not strictly comply with the continuous infusion.Regarding the investigation of nurses’ knowledge about the importance of the pharmacological concept of that drug infusion 57.14% partially dominate the pharmacological importance of the concept of continuous infusion. In conclusion the development of protocols that specify the need for continuous solutions exchange can help significantly the nursing care assistance.


El objeto de este estudio fue evaluar el conocimiento de los enfermeros sobre la importancia de la infusión continua decatecolaminas. Se trata de un estudio exploratorio descriptivo llevado a cabo en la Unidad de Cuidados Intensivos de un hospital universitario de Fortaleza-Ceará. Se realizaron entrevistas con 49 enfermeras. La mayoría, el 80,4% (40), no era especialista en terapia intensiva. Los dispositivos más utilizados para la infusión de catecolaminas eran la bomba de infusión y el catéter (93, 88%). 54,34% de los entrevistados informó que no seguía estrictamente la infusión continua.En cuanto a la investigación del conocimiento de los enfermeros sobre la importancia del concepto de infusión de medicamentos el 57,14% de la muestra indicó dominar parcialmente la importancia farmacológica del concepto de infusión continua. Se concluye que la elaboración de protocolos especificando la necesidad de cambiar las soluciones podría ayudar significativamente a la atención de enfermería.

ABSTRACT

La sepsis es la causa más común de muerte en pediatría. Conocersu manejo adecuado tendrá un fuerte impacto en la morbimortalidad.El pediatra que toma contacto inicial con estosniños cumple un rol fundamental en la cadena de acciones que suele iniciarse en el Área de Emergencias y finalizar en Cuidados Intensivos. En los últimos años se elaboraron estrategias para cumplir objetivos escalonados. Las metas por alcanzar son clínicas, hemodinámicas, bioquímicas y de oxigenación. La correcta interpretación de cada componente afectado permitirá la intervención más apropiada. La administración intensiva de fluidos y la precoz indicación de inotrópicos resultan pilares esenciales. Este trabajo resume las principales intervenciones en el Área de Emergencias.

ABSTRACT

Nucleus accumbens is proposed as one of the centers in the neural circuitry involved in the regulation of feeding and drinking behaviour in rats. Injection of dopamine and angiotensin-II into this nucleus has been documented to affect water and food intake in rats. Reports on the effect of intracerebral injection of catecholamines on feeding and drinking behaviour in animal models are conflicting. Therefore, in the present study the effect of adrenaline and noradrenaline injected into nucleus accumbens on food and water intake in rats was assessed. 24 h basal food and water intakes were recorded in Wistar rats and were found to be 12.3 +/- 0.46 g and 21.7 +/- 1.03 ml respectively. Stainless steel cannulae were implanted stereotaxically into the nucleus accumbens. Four different doses (0.1 microgram, 0.5 microgram, 1 microgram, and 2 micrograms) of adrenaline and noradrenaline were injected into the nucleus accumbens through the implanted cannulae in different group of animals and their 24 h food and water intakes were recorded following these injections. No change in food and water intake was observed following the administration of different doses of adrenaline. A significant increase in 24 h water intake reaching a maximum of 28.88 +/- 1.45 ml at 1 microgram dose, without change in food intake was observed following administration of different doses of noradrenaline. The noradrenaline-facilitated water intake was blocked when noradrenaline was injected following injection of phentolamine, an alpha-receptor blocker. The bilateral lesions of nucleus accumbens resulted in a significant and sustained inhibition of water intake (16.61 +/- 0.67 ml) without change in food intake. These observations suggest that noradrenaline facilitates water intake without affecting food intake when injected into the nucleus accumbens in rats and the dipsogenic effect of noradrenaline is mediated by alpha-receptors. Adrenaline does not affect these ingestive behaviours when injected into the nucleus accumbens in rats.

ABSTRACT

Objetivo: revisar as bases teóricas das abordagens terapêuticas voltadas para a ressuscitação volumétrica do lactente em choque e o processo evolutivo que levam ao suporte farmacológico à circulação. Métodos: Foram obtidos referências de busca computadorizadas na Biblioteca Nacional de Medicina (Medline), artigos recentes de revisão e arquivos pessoais. Resultados: Choque e choque séptico são importantes causas de morbidade e mortalidade em crianças criticamente doentes. O choque séptico é um processo de doença extremamente complexo que conduz à disfução múltipla de órgãos. Sua apresentação pode ser sutil ou catastrófica. Um mmanejo bem sucedido depende de uma compreesão do fisiopatologia da síndrome e exige uma ressuscitação rápida e apropriada. O tratamento freqüentemente requer correção agressiva de déficit de volume, manutenção de perfusão adequada com inotrópicos e vasopressores, ventilação mecânica e outras terapias. Conclusões: O manejo do choque nas crianças representa um desafio para o pediatra. Instabilidade hemodinâmica e respiratória devem ser antecipadas. Sau manejo requer intervenção multissistêmica e cuidadosa

ABSTRACT

Obesity is highly prevalent and has several adverse effects on health. Its treatment is thus warranted and must aim to modify dietary and physical activity habits. The opinion of this association is that anorexigenic drugs with cathecolaminergic action (diethylpropion, phentermine, mazindol and phenylpropanolamine) or serotoninergic action (fenfluoramine and fluoxetine) may be used in moderate or severe obesity (BMI >30 kg/m2) after a complete clinical assessment and in the context of an integral medical treatment. This association recommends a close surveillance of the use of these drugs, specially when formulated as non-propietary prescriptions
